The primary goal of orthokeratology is to correct refractive errors, such as myopia (nearsightedness), by gently reshaping the cornea while the person sleeps.

- Lens Wear at Night: Ortho-k lenses are worn overnight during sleep. These lenses apply gentle pressure to the cornea, causing temporary changes in its shape.
- Corneal Reshaping: The lenses flatten the center of the cornea for myopia, helping light to focus properly on the retina. This reshaping effect lasts for a limited time, typically requiring nightly lens wear to maintain the correction during waking hours.
- Daytime Freedom from Glasses or Contacts: After removing the ortho-k lenses in the morning, individuals can experience clear vision throughout the day without the need for glasses or contact lenses. This temporary correction can be particularly appealing for those who prefer not to wear corrective lenses during the day.
Orthokeratology is often used as a myopia control strategy, especially in children and adolescents, to slow down the progression of nearsightedness.
